Instructions:

The following survey instrument is designed to collect data from SIM sites on key indicators of their progress in achieving the desired outcomes resulting in the integration of SIM as the organization’s operational philosophy. The performance indicators include measures in a range of key functional areas of operations that help assess progress including those impacting safety, security, inmate health and well- being, inmate behavior, program participation, as well as staffing levels, behavior, and training. Inmate population data are also collected as a means to provide comparative data over time. NIC will utilize this data to assess the effectiveness of the assistance provided through the SIM Initiative and may adjust its offerings accordingly. NIC will also provide some preliminary analysis of the data submitted by the SIM site and share back the results along with the source data in spreadsheet format which will facilitate further analysis by the jurisdiction based upon their interests and needs. As an individual designated to complete the survey for your organization, you are to complete the survey to the fullest extent possible for each of reporting period and submit the completed survey.
